Information needed for assistance |
| When
meeting with one of our case workers, clients are requested
to have the following information: |

- Legal
name/address/telephone. Number of people in household and
names/ages.
|
| |
- The
situation... i.e.., the situation which created their current
need ... what the immediate or short-term outlook is...
etc.
|
| |
- Whether
there is household income and how much on a monthly basis.
Guidelines for assistance are established based upon recognized
standard poverty levels, but each situation is evaluated
on a case by case basis.

| |
- Other
pertinent information, as needed for guidance or referral
to outside agencies or special contacts that also help.
